When you book a New Patient appointment, we will email you our New Patient Inake Form. No need to print this out, simply fill out the form and we'll take care of the rest!

MN Care, Medical Assistance, and State Subsidized Private Plans
Please note, State Funded plans allow 1 exam per year regardless of specialty. Our initial exam may or may not be covered depending on any care you have received from medical doctors or providers during the year. Because of this, we collect the initial exam fee ($65) at your first visit. If your exam is covered by insurance, we will credit the $65 to your account which can then be used towards co-pays, services or products we sell.
